Model Release - Portfolio Shoot

• I, Ms./Mr. _______________________________ (model/guardian) had approached the photographer to photograph a paid promotional model portfolio for me/my ward. I, _______________________________ (model/guardian on behalf of the said minor) hereby release the image copyrights and all my rights in the negatives, slides, prints and digitalized format of the said photographs in favor of the photographer who shall be absolutely entitled to promoting his career by publishing, advertising, utilizing in any manner or media the said photographs and drawing/s therefrom and other reproductions or adaptations thereof, either in part or complete, alone or in conjunction with any wording for publicity or otherwise throughout the world, and these reproductions and adaptations in any format shall be deemed to represent an imaginary person, and shall have no right at any time to claim any share in the remuneration the photographer may derive therefrom.
  
• I shall have no objection to the photographer helping me in my modeling career through any media/forums that he feels could be good in promoting me to the best of his knowledge, intentions, and judgement by publishing my pictures, specifications, and contact information. I will decide the finalization of any proposals received from such an exposure and remunerations for the same.
  
• I want him to publish in any such forums my contact information that is mentioned at the bottom, pictures, and specifications that he thinks fit, so that any proposals of work can be handled directly by me.
  
• I do not want him to publish in any such forums my contact information that is mentioned at the bottom except my name and pictures with my specifications, as I want him to filter any proposals of work to the best of his knowledge and judgement but the final decision of acceptance rests with me.
  
• (_) I declare that I am a major, (D.O.B. _______________ - Age: _____________years), and all pictures taken during the photo session were with my full consent and agreement and I am of legal age to take such decisions. • (_) I declare that I am the legal guardian of the minor, whose D.O.B. is _______________ - Age ________ years and I am ___________________ of the above-referred minor, and all pictures taken during the photo session paid by me were with my full consent and agreement. • I declare that I shall at no time at the behest of anybody initiate any legal actions as I had assigned the photographer for my portfolio development and all images during the photo session were taken with my full participation, consent and understanding.
• We mutually agree on one of the following arrangements, where the photographer will give, after my full participation in the selection process, as mentioned under:
  

• I, (photographer's name), hold complete creative copyright of all the photographs, but have no objection to the above model posting and publishing any images from the photo session in promoting his/her modeling career in any forum or media that gives him/her a mileage. These may not be commercially exploited in any forum or medium in the present or in the future. It would be appreciated, if photo credits could be published in such postings and publications.
